A to E Assessment for Deteriorating and Acutely Unwell Patients
"...the 80 assessment is a fundamental tool used to provide a quick assessment for a patient who is critically ill it follows an order going through the potentially life-threatening problems giving priority to the most severe issues all right so the stands for Airway and what we're doing here is basically checking that there's no obstruction but also evaluating the likelihood that the patient will be able to maintain the airway if they deteriorate further there are several things you can do to assess the airway you can watch the patient and look for signs of an obstruction such as paradoxical chest or abdomen movements cyanosis which is a blue discoloration of the skin or use of accessory muscles you can also listen for sounds for example if the patient is talking that a fairly conclusive sign that the airway is obstructed however if you hear sounds extruder which is a high pitch sound mostly in inspiration or even a wheeze these sounds are indicative of a partial obstruction so what happens if you hear nothing you're either have to try and hear the breath sounds using a stethoscope or you'll have to try and feel for it the way you do this is by putting your hand or your ear next to the patient's nose or mouth and feeling for the air flow causes of an airway obstruction include the patient's tongue a foreign body swelling and even fluid it's such as vomiting or blood so what do we do about it well if it's obstructed the patient can rapidly deteriorating die or suffer hypoxic damage to organs such as the brain and heart so the first thing you do is you get help depending on the cause the patient may be moved into the recovery position so on this side you may perform movers like back slaps or the abdominal thrusts in order to help dislodge a foreign body it's patient ventilating and normal respiratory rate is between 12 and 20 breaths per minute hide and 25 is a marker that the patient may deteriorate quickly we would also look at the depth and the effort of the breathing are they using the accessory muscles is the chest expansion symmetrical is the trachea displaced or is the hyper resonance investigations include the use of a pulse oximeter so measuring the oxygen saturation generally we aim at for to 95% with a minimum of eighty eight percent however you need to be careful in COPD patients not to overdo the oxygen therapy because you can end up suppressing their breathing life-threatening causes of breathing issues include acute severe asthma pulmonary edema and tension pneumothorax the treatment again depends on the cause oxygen is given in most cases if no obstruction is present then non-invasive ventilation..."

Nephrotic syndrome is a collection of signs and symptoms that result from damage to the kidneys and is often confused with nephritic syndrome in fatigue syndrome there is a substantial amount of protein being lost through the kidneys in the urine defined as more than 3.5 grams per day this leads to hypoalbuminemia a low level of albumin in the blood as albumin is the most abundant protein normally in the blood these two are defining features in nephrotic syndrome nephritic syndrome is different in that there is less proteinuria but also the presence of hematuria and red blood cell or white blood cell casts in the urine hypertension and oliguria which is a reduced urine output typically between 80 and 400 ml per day the unit of the kidney is the nephron and normally in the glomerulus of the Nephron there is a specialized membrane that forms the filter made up of a fenestrated endothelium the Maryland basement membrane and the foot processes of podocytes which are cells that wrap around the capillary giving this additional filtration layer all together these structures normally act as a filter and prevent large molecules from passing through into the Bowman's capsule and renal tubules they have a net negative charge which may mean that they can repel other negatively charged molecules albumin is negatively charged in the fatigue syndrome there is sufficient injury to these structures to change the permeability and allow albumin and other molecules to pass into the urine leading to hypoalbuminemia the signs and symptoms linked to hypoalbuminemia which includes peripheral edema and fluid overload particularly in children this can be evident as facial swelling but can also occur in adults particularly around the eyes fluid overload can mean weight gain peripheral edema and even the development of ascites or pleural effusion which can manifest as shortness of breath known as this near the reduction in the blood levels of albumin causes the oncotic pressure of the blood 24 meaning fluid will more readily leak into the surrounding tissues causing edema this is sensed as hypovolemia because less fluid is in the vessels which then triggers the renin-angiotensin-aldosterone system causing retention of salt and water this is known as the underfill hypothesis while another is the

"...first question is do you ever see patients who experience inner Tremors can you address what this might be all about and I would say yes we can address what this is all about we see a lot of patients with a variety of some Tremors and inner tremors are something that have a specific medical understanding for us yeah and the internal Tremors can be a number of things you could be anxiety it could be an autoimmune response your thyroid it could be hyperthyroidism it could be something like a pheochromocytoma which is where your adrenal gland has a tumor on it and makes to me adrenal hormones so all of those are possibilities for the internal Tremors we frequently see it with autoimmune thyroid disease but we see a lot of so that so that's kind of our take on anxiety a lot of anxiety those are two things I would go to those two things first I don't know yeah okay but yes can you have fibromyalgia this is a great question because it's kind of fiber understanding of fibromyalgia can you have fibromyalgia not have irritable bowel syndrome and not hurt when someone hugs you or touches you great question the understanding of what it fibromyalgia is evolve and and we've stayed on top of that Gates has done continued to stay on top of the research so I think this is an interesting question to answer for people who are confused yeah like me right so if I were myalgia we had the 1990 criteria by the American College of Rheumatology and that criteria set forth that if you have tenderness at a certain number of the trigger points 11 out of the 18 I know they team basically then you had fibromyalgia to produce 11 out of 18 and then you had other Associated symptoms of brain fog and fatigue whereas the new criteria the 2010 American College of Rheumatology criteria that came out and it's predicated on not having the trigger points you don't have to be able to termed a low zdenek allodynia constitutes pain with a normally non painful stimulus so like somebody giving you a hug so you don't have to have that pain response when somebody gives you a hug and you don't necessarily have to have IBS either in order to be diagnosed with fibromyalgia now it can't be another condition that's causing the pain it can be associate with other pain but you have widespread pain so pain and many areas of your body there's a way to document that you have other disabling symptoms of fatigue and brain fog and the fatigue and brain fog half of these severe enough to be above a certain score level for you to be diagnosed with fibromyalgia so it's really more predicated on widespread pain fatigue and brain fog and then it also factors in other Associated symptoms like IBS but you don't have to have IBS you don't have to have trigger points to have fun that you can have other gut problems like I have Celiac can write or you could just have non-celiac gluten sensitivity.."

...we're just going to answer some of the questions that we haven't had a chance to answer over a long period of time and really now and then we will answer select questions that seem to be that we think a lot of people out there might be asking based on what we hear in the histories that come in based on what we hear from people who have the types of conditions that we address so the first question we're going to address today is pernicious anemia autoimmune seem to me that that one could be pretty well easily fact figured out on the internet Canada cause severe pain and tummy issues you described I think pernicious anemia is autoimmune but I think it's a very interesting thing to maybe expand on a little bit I just things that might be connected to and so is it autoimmune and it caused severe pain and tummy issues through describe the tummy guy will answer that now so relative to pernicious anemia technically it's autoimmune gastritis where the immune system is attacking your stomach with pernicious anemia specifically we have immune cells that attack intrinsic factor intrinsic factor is the product made in your stomach cells that binds the V12 so you can absorb your B12 but in that process a lot of patients have autoimmune gastritis may also have their immune system killing the parietal cells of the stomach as well and in the course of that means basically your immune system is killing your stomach just like if your immune system is killing your joints it would probably hurt a lot of patients who have autoimmune gastritis do have gastritis and stomach pain and we pick up this entity pretty frequently it's not very commonly tested for in the medical world unless you are egregious lie low and b12 that's been our experience and you also have to have a serum gastrin done as well because gastrin is a hormone made in your stomach and when your stomach is breaking down gastrin goes up that's one way to detect pernicious anemia or ottoman gastritis the answer the...
