For preoperative patient evaluation, the Task Force focused on preoperative imaging, preoperative laboratory evaluation, and preoperative ophthalmologic evaluation. For primary treatment, this guideline addresses surgical resection; medical therapy; radiation therapy; the natural history of untreated tumors; surgical methodologies such as endoscopy, microscopy, or craniotomy; and intraoperative adjuncts like neuro-navigation, CSF diversion, or intraoperative imaging. For residual tumor treatment, the Task Force evaluated radiation versus observation. Additional topics addressed in this guideline regarding postoperative patient management, include the frequency of postoperative imaging, postoperative endocrine evaluation, and postoperative ophthalmologic evaluation.

One objective of this review is to determine preoperative diagnostic studies that predict increased risk of reoperation or SSI in patients with diabetes. In addition, the published literature was assessed to determine if an increased BMI correlates with an increased risk of reoperation or SSI. Finally, the impact of preoperative smoking and risk of reoperation after spinal fusion was evaluated and if smoking cessation decreases risk.

The use of bilateral anterior limb of internal capsule (ALIC) DBS for OCD was approved by the US Food and Drug Administration (FDA) under a humanitarian device exemption (HDE) in 2009.9 Following this HDE approval and publication of the 2014 CNS guidelines, there have been additional published studies reporting the use of DBS for OCD, although most do not meet criteria to be included in this guidelines update. The purpose of this update is to review the literature following publication of the original guidelines and to update the recommendations as appropriate. Based on the availability of new literature, the current guideline is a major update resulting in modification and update of prior recommendations as well as an updated discussion.

Surgery is recommended for brain metastases that are large, have significant perilesional edema, result in neurological deficits, and present with uncertain pathology. In addition, surgery provides tissue diagnosis, when needed. Smaller targeted craniotomies and an emphasis on minimizing postoperative deficits have led to faster operations and discharge a few days after a craniotomy. Given the limitations of radiation therapy and other targeted therapies, surgery plays a critical role for patients, the timing of which is discussed in this guideline.

The efficacy of bilateral deep brain stimulation (DBS) for the treatment of motor symptoms and levodopa-induced dyskinesias in Parkinson’s Disease (PD) patients is well established. However, the effectiveness of selecting different stimulation targets is less clear. While there is evidence to suggest that the subthalamic nucleus (STN) and globus pallidus internus (GPi) are effective targets, when combined with best medical treatment versus best medical treatment alone, the circumstances in which one target should be selected over the other are still disputed. It is also unknown whether STN and GPi DBS induce similar benefits. In this guideline, the authors systematically review the literature and grade the quality of evidence for the use of STN and GPi in DBS in patients with PD.
