AHRQ Misdiagnosis Study Sounds False Alarm About America’s Emergency Rooms

Doctor Jesse Pines is the National Director of Clinical Innovation at US Acute Care Solutions. Dr Pines most recently served as Director of the Center for Healthcare Innovation and Policy Research and a Professor of Emergency Medicine at the George Washington University School of Medicine and Health Sciences.  He has been a Professor of Health Policy and Management at the GW Milken Institute School of Public Health and also was the principal investigator for Urgent Matters, a program that disseminates information on best practices in emergency care for eight years. The Problem:On December 15, the New York Times published an article entitled "E. R.  Doctors Misdiagnosed Patients with Unusual Symptoms."  The article was based on a study published by the Agency for Healthcare Research and Quality that was entitled "Diagnostic Errors in the Emergency Department: A Systematic Review."The study has 15 authors with the lead author being David E. Newman-Toker, MD, PhD. In addition to the 15 authors there are seven Key Informants whose job was to provide input on the priority areas for research and synthesis. In addition, there was a panel of six Technical Experts whose job was to design the studies questions and methodology. Finally, there were four Peer Reviewers - Dr Pines was both a Technical Expert and Peer Reviewer of the paper. The paper is 744 pages long with 222 references.  The study found 279 citations related to the topic. One prominent source of concern of emergency physicians was the fact that it was assumed that emergency care provided in some countries was comparable to that in the United States.  The authors included studies if they were conducted in the United States, Canada, United Kingdom, Western Europe, Australia, or New Zealand because the advisors to the study felt these countries and regions had roughly comparable systems of ED care to those found in the United States.  U.S. emergency physicians, most Board Certified in EM, refute that assertion and push back immediately. "In addition to making misleading, incomplete and erroneous conclusions from the literature reviewed, the report conveys a tone that inaccurately characterizes and unnecessarily disparages the practice of emergency medicine in the United States," Dr. Christopher S. Kang, the president of the American College of Emergency Physicians, said in a statement."As with all medical specialties, there is room for improvement in the diagnostic accuracy of emergency care," Dr. Kang added. "All of us who practice emergency medicine are committed to improving care and reducing diagnostic error.""While most medical specialties have similar training in Western nations, emergency medicine does not," he said. The Study Sought the Answers to Three Key QuestionsWhat clinical conditions are associated with the greatest number and highest risk of ED diagnostic errors and associated harms?Overall and for the clinical conditions of interest, how frequent are ED diagnostic errors and associated harms? Overall and for the clinical conditions of interest, what are the major causal factors associated with ED diagnostic errors and associated harms?Visit us at ccme.org and watch this episode on Youtube at:https://www.youtube.com/watch?v=-ccQbfcurHc

Risk Management Monthly - March '18

Dr. Bitterman wrote a piece in the October 2017 issue of ACEP Now about the circumstances surrounding the largest monetary penalty ($1.295 million) levied in the history of EMTALA, against AnMed Health in Anderson, SC (the issue was also addressed in the December 2017 issue of RMM).  The claim by CMS and the OIG against AnMed cited failure to appropriately screen and stabilize psychiatric patients in the ED, as well as "inappropriate transfer" of "unstable" psychiatric patients.  The cases involved patients with known long-term psychiatric conditions, Who can screen and stabilize psychiatric patients?, When is psychiatric patient stabilized?, What To Do At Your Hospital Based on the AnMed Case Since 2007, the 80-Minute Monthly Podcast Designed to Keep Emergency Clinicians Out of the Courtroom!  Doctors Greg Henry and Rick Bukata provide listeners with a no-holds-barred monthly podcast consisting of critical reviews of closed claims, discussions about common clinical mistakes, tips for safe charting, reviews of important articles, periodic interviews with risk management authorities, answer listeners questions and lots more.  To learn more, visit ccme.org/riskmgmt/

Managing Migraine

Migraines are no longer considered a vascular headache and now considered a neurologic disorder of dysfunctional nociceptive processing.  Diagnosis is completed clinically with no lab or imaging usually needed.  Treatment includes - Antidopaminergics (Metoclopramide & Prochlorperazine) - Triptans (Sumatriptan) - Nonsteroidals (Ketorolac).  Consider steroids to lessen the risk of recurrence.  Opioids should be avoided and are last line. Most patients don’t need specialist referral.  To learn more, visit https://courses.ccme.org/course/em-prep

Does it pay to advertise?  Do freestanding EDs increase Medicare spending?  We also look at converting Urgent Care centers to freestanding EDs, "surprise" medical bills, the high cost of sending an emergency department bill, and ED utilization of APCs.The focus of the Innovations in Emergency Department Management Course is on leadership, management, operations, logistics, strategic planning and healthcare reform. The Geriatric Emergency Department

Annually, almost half of American adults over age 65 have an ED visit. Although adults over age 65 represented only 15% of ED visits in 2014, they accounted for 48% of hospital admissions from the ED. For almost 30 years, emergency medicine research has demonstrated that increasingly busy physicians, advanced practice providers, and nurses miss prevalent geriatric syndromes like delirium, dementia, short-term fall risk, and polypharmacy – the consequence of which is preventable ED returns compounded with diminished patient satisfaction scores. In response, geriatric emergency medicine leaders have emerged since the mid-1990’s and in 2013 published guidelines intended to “geriatricize” EDs of all sizes and geographic locale, guidelines that were subsequently endorsed by multiple emergency medicine and geriatric organizations’ boards of directors (https://tinyurl.com/GEDGuidelines19). Watch Dr. Klauer present this talk @ https://www.youtube.com/watch?v=yHOjh63wV8MJoin us live in 2021 for a focused, evidence-based course that’s designed to update you on over 30 leading-edge EM topics......8 TOP Destinations!!https://courses.ccme.org/course/ema/locations
