#106. Rewind Episode 5: The Arrival Fallacy
Here is a rewind episode of one of our favorites. “Someday when I become a resident, it will be better. I’ll be making money. “ “One day, as an attending, things will be so much better than they are right now. No one will be able to tell me what to do!” “When the kids are a little bit older, things will be easier.” I am here to tell you that if you think when you accomplish your next goal life will automatically be better, you are likely suffering from an arrival fallacy. Here is how to snap out of the matrix, and find the reality you really want today. Love the show? Heck, want to work with me? See how we can here! www.thephysicianphilosopher.com/coaching

#104. Creating Healthy Boundaries With Technology
Years ago, I transitioned from physician burnout to entrepreneur burnout in my online business. And it was primarily from not having healthy boundaries with technology. When I first started out in entrepreneurship, I worked 60 hours a week in medicine plus another 20-30 hours a week on my blog and business. Business culture indoctrination often means we find ourselves believing we need to hustle 24/7 to get results, but it doesn’t have to be that way. What’s really important about today’s episode is understanding how we can use technology not to worsen our constant availability – which can lead to burnout – but instead how to use that technology to set boundaries.

#103. How Much Money Should Doctors Save
One thing we can agree on about today’s topic is this: doctors historically are under-savers. But when it comes to physician money management, exactly how much doctors should be saving is hotly contested. Lisha and I spent 15 minutes debating our perspectives before we even hit record! So we know you’re going to land with a variety of opinions. Of course, how much and how you save are going to depend on your own financial goals and your current situation - the status of your student loans, whether you’re married or have kids or plan to start a family, when you want to retire, etc. So by the end of this episode, we’ll give you two different options for figuring out how much money you should save - regardless of your situation. https://thephysicianphilosopher.com/ACE

#102. The Problematic State of Medicine
With today’s episode, we’re doing things a little differently. Instead of a podcast, you’re getting a sneak peek of the audiobook version of my latest book, “Determined: How Burned Out Doctors Can Thrive in a Broken Medical System.” Yes, we’re talking burnout – a topic very familiar to you as a medical professional. It’s messy, it’s layered, and it’s part of the problematic state of medicine. Burnout is twice as high in physicians versus the general population. Not only is that statistic alarming, but the potential consequences of burnout on medical professionals are staggering. Higher rates of substance and alcohol use, depression, and suicide are not uncommon problems in medicine. On the systematic front, burnout has been linked with higher physician turnover, worse patient care and outcomes, decreased patient satisfaction, higher healthcare costs, and increased risk for major medical errors. Burnout is costly for physicians and for the patients under our care. But as Albert Einstein once wisely said, “We can't solve problems by using the same kind of thinking we used when we created them.” In this week’s episode of The Physician Philosopher podcast, I’m sharing my audio recording of the first chapter of my book. You’ll Learn: The top 5 reasons physicians feel burned out Why we need greater focus on personal and professional autonomy in medicine How compassion serves as a key factor in burnout recovery and systematic change The role of purpose and how it deepens our professional experience Take a listen on your favorite podcast listening platform and leave a comment. #101. Paying Off Debt or Investing for Physicians
I can’t tell you how often other physicians ask me, “Hey, I understand the math, but I just don't know: should I be paying off my student loans? Should I be investing and maxing out my 401k, or should I be paying off a car?” There’s advice out there that says you should invest at all costs, that anyone who doesn't put all their money towards investments is crazy. Then, at the other end of the spectrum is the school of thought that says you shouldn’t do anything else until you pay off all your physician debt. As with most nuanced subjects, it’s not black and white. The right path – for YOU – depends on a lot of different factors. When figuring out whether to pay off debt versus investing, it requires more than just math – because humans aren't robots. https://thephysicianphilosopher.com/ACE

#100. 3 Doctors Who Changed Their Life Through Physician Coaching
Given that this is the 100th episode of this show, I thought it would be pretty cool to share the stories of those who have listened to this podcast and had their lives changed as a result. And today is doubly-special, because The Alpha Coaching Experience (ACE) is finally open again. It’s been six months since we opened the doors last, and we only open the program twice per year. So, instead of me telling you about the impact ACE has had on doctors, I thought I’d let three doctors who have gone through the physician coaching program share their stories. Here’s how they overcame their situations to create the time, financial freedom, and mindset shifts they needed to practice medicine on their terms. HTTPS://thephysicianphilosopher.com/ACE

#97. Practicing Medicine On Your Terms with Dr. Vikram Raya
Physicians that have carved out a life they love through entrepreneurship have one major thing in common: they work on their mindset. I once had massive skepticism about mindset work and coaching. But there's a reason why people say that when it comes to personal finance, it's 20% math, 80% mindset. If you’ve been around here for a while, you’ve come to hear that phrase over and over and over. But until we take that leap of faith and try it, we don't quite understand what it means when people say they're working on their mindset, or the place it has in the entrepreneur journey. Dr. Vikram Raya’s journey to becoming a physician entrepreneur is a prime example of this.

#96. What Stops Doctors From Making a Change
Why are so many intelligent, educated, and hard-working physicians struggling? Why don’t they just take themselves out of their current situation and find something better? The answer is fear. So, in this episode, I wanted to discuss some of the common fears that I see among our physician clients in ACE and others who reach out to me through email and social media. And what you can do to overcome those fears.

#95. Should a Doctor Go Into Academics or Private Practice?
I tell my residents that academics is about depth and private practice is about breadth. In a lot of specialist fields, the private practice opportunities can provide some of that breadth. What you choose really depends on your preferences and how you feel you can work best in a given environment when weighed against all the factors that are important to you. But at the end of the day: when choosing a job, make sure you think through the worst-case scenario before saying “yes”. You’ll Learn: A tool I use with my coaching clients to help you think through any job decision An in-depth perspective about the benefits (and drawbacks) of going into academics as well as joining private practice Physician-specific finance and career factors you should consider as you’re making your job decisions Want to learn more about money? #94. How Virtual Assistants Can Benefit Doctors
I'm a big fan of delegating tasks and freeing up my time so that I can focus on what I need to focus on in my business. And finding work-life balance (or life-life balance as some people like to call it) is one of the most common things doctors want. But how do we go about finding that? Enter virtual assistants for doctors. They could be just the solution for many doctors on the road to burnout by assisting with scheduling, pre-authorizations, paperwork, and more. If you're a doctor who doesn’t know what a VA is or how they can help you get back some of your time (or potentially make you more money), this one’s for you.

#92. How Burned-Out Doctors Can Thrive in a Broken Medical System (Determined Book Release)
Physicians often reach out to me for advice about whether they should quit being a physician. I have a hard time giving advice on that subject because it seems like there are really only two options. The first option is to tough it out and finish what you started. The second option is to follow your dreams, even if they lead you away from medicine. So how do you know which path to take? Chelsea Turgeon wrote the book on it - literally. #66. What Communism and Physician Culture Have in Common
What does healthcare have in common with communism? And why does this cause major problems and lead to more burnout and lower profit margins? Burnout in medicine is expensive. Each year it costs us billions of dollars. What if the healthcare industry can transition to a different model of care that centers people over profit? When leaders learn to choose people first, the results are positive and endless. In this episode, I spend some time explaining a fundamental concept that will help physician leaders make a decision that not only reduces burnout but provides a return on their investment.

#65. The Difference Between Simple and Easy
Have you ever wondered why you have a hard time sticking to your goals? Maybe you think it is willpower or motivation? Maybe you think you end up not having enough time to put in the work required? What if I told you none of that is true? What if I told you that most people think these are the reasons they can’t reach their goals and these thoughts continue to put them on a path of “I can’t do this.” What you will learn in today’s episode is the ONE thing you need to know to reach your goal. In fact, it doesn’t have to be hard. This one thing will make it easier and more fun. Let’s get started.

#64. The Importance of Financial Freedom and Mindset with Dr. Daniel Shin
One of the pillars to physician freedom and finding balance in your life as a doctor is money. It’s no secret that financial freedom when coupled with mindset can help you build a life within medicine that you love. One that doesn’t take all of the time you have away from the things you love, but supports it. So in this episode I am talking about personal finance, part-time work in medicine, and finding a work-life balance as a husband, father, physician, and entrepreneur with an Alpha Coaching Experience client, Daniel Shin. Learn how coaching played a part in helping him overcome burnout and allowed him to find work-life balance he needed. #63. Creating More Time and Money as Doctors
Setting boundaries is the best way for doctors to increase the time and money they need to create their dream job in medicine. In order to reclaim your personal and professional autonomy, it requires doctors to set boundaries with your mindset, money, and time. But how do you set boundaries and keep them? What does this require? What is holding you back from setting boundaries in multiple areas of your life? In today’s episode I am going to dive into why the solution to a more balanced life as a doctor is setting boundaries and the one tool that can help you. We will be diving deeper into this solution and tool in our upcoming free 2 day live event, The Rise of The Self Determined Physician on January 25th and 27th. How many times have you been tired, but you’ve been told to put your head down and keep pushing? Self-compassion does not come easily to many physicians, but it is one of the most important things we can do for ourselves, families, and patients. Have you ever heard someone say, “I’ll sleep when I’m dead”? Have you ever said that yourself? And the images of doctors being “good soldiers” who put their head down and get the work done, regardless of how bad things are. If you are a hard-working physician, you know what I am talking about. Rest feels counter-productive, and our culture in medicine teaches us the same. But this is a toxic notion and something that I am going to address today on this podcast. Everything in life has trade offs. Unfortunately, most of us don’t realize that this is happening, and so these trade offs happen unintentionally. We say “yes” to all sorts of things, and don’t realize that this means we are saying “no” to many others. If you aren’t clear about what your priorities are, someone or something else will set them for you. Maybe it’s your job, your boss, or someone else in your life. On today’s episode, understand why the idea that you don’t have a choice is just a story you have learned and how a 1967 study on German Shepherds proves this. #55. The Therapeutic Index of Physician Thought Work
In medicine we talk about the therapeutic index or therapeutic window for the drugs we prescribe. Every medication has both a potential benefit and potential risk. The therapeutic index describes the safety of the drug. The goal, of course, is to find the therapeutic window of a drug where we maximize efficacy while limiting side effects. In this podcast, I dive into the therapeutic window of coaching (yes, one exists). Because it is important to understand how coaching tools can be used to create great change or can be used to inflict pain on yourself. I want to talk about how to avoid the later and use these tools correctly. Are you a physician looking to get coaching of your own? Did you know that you have the power to create more time in your life? Time that allows you the work-life balance you are desperately seeking. Mom (and Dad) guilt is a real thing that many parents experience. #50. Is Coaching the Cure to Physician Burnout and Moral Injury?
So many doctors and physicians would agree that the medical system is broken. The system has been built to accommodate so much paperwork and those that hold the money and doctors are at the mercy of it all. #49. Knowing Your Self-Worth as a Spouse, Parent, & Physician
The number one thing people talk about when approaching death is the idea that they wish they had been unapologetic about who they are and what they wanted in life. So often as physicians we seek the validation of others rather than unapologetically seeking our own validation. The current system of medicine doesn’t help either. This system quickly turns doctors into external validation junkies. Our entire self-worth can become attached to who and what other people think about us as physicians. But what if you knew your self-worth and made decisions from what is most important to you? This idea (and how you get there) is what we are going to dive into on today’s episode. #47. Cell Phone Buffering and Physician Families: Lessons from Napolean
When you find yourself reaching for your phone when you are off the clock to check your email. Or drinking not because you are enjoying that IPA, but because you are avoiding something. Or when you begin to think about all the people that need you to respond to their text within minutes, you might be buffering. Buffering is very common in our culture today, but it isn’t a way to live life that will help us feel like the person, parent, and physician we want to be. Listen to today’s episode to learn what Napolean taught us years ago and might just help you uncover the real reason you are buffering today. #36. Working More Shifts is NOT the Answer to Financial Freedom
Why do you want financial independence so quickly? Many doctors think that by working extra shifts or by seeing more patients they will get to their financial freedom sooner. #34. The Physician Should Come First (Not the Patient)

I'm here to tell you that this way of thinking is backward and if we take care of doctors first, then everyone benefits.

The more work you do, the more money you earn, right? That's how we're taught to think, and also why a lot of doctors work themselves into burn out. #26. Why doctors Benefit from Professional Coaching
Does this sound familiar to you? You finish your medical training thinking your burnout and overwhelm will disappear once you are an attending physician and have a salary. #25. 5 Keys to Financial Freedom through Physician Side Gigs
Financial freedom can be obtained by saving enough to retire, producing enough income to support your lifestyle, or a hybrid of both. Only 4 years out I’m financially free from medicine, and practice medicine now because I want to and not because I have to. For some people though, this financial independence may seem forever out of reach. We all want the autonomy to do what makes us happy in our lives, but for many doctors they may be tied to financial restrictions that take the joy out of their work. If you resonate with what I just said, then maybe it's time to create a non-clinical source of income, and I'm not talking about "jobby" (Job+Hobby). Today I break down my top 5 keys to help you create non-clinical income, and start earning your financial freedom, too. If you are interested in coaching on how to start your Online Physician Side Gig, the Alpha Coaching Experience is officially on sale from 2/13 until 2/22. Included is 12 weeks of both life coaching and business coaching. Whether you are a busy doctor looking to deal with burnout and overwhelm or building the online business of your dreams, you can enroll at https://thephysicianphilosopher.com/alpha . Spring Enrollment ends 2/22 at midnight!

#24. Quid Pro Quo & Disappointing Relationships
Quite often, we get mad at others for reasons that are out of their control. In our minds the problem is that they didn’t meet our expectations. However, the source of the frustration isn't the reality of things happening or not. It's actually that reality doesn't match up with our expectations. This can cause unhealthy relationships both at work and with your family. Today we are going to discuss how harmful conditional love sets you up for disappointment with your co-workers and family. Instead, I want you to choose to love unconditionally and appreciate the people in your life. Also, if you want to learn more about how to control your feelings in and outside of your job, I am hosting a webinar titled Physician Burnout: 3 Essential Concepts to Finding Freedom on 4 upcoming dates starting February 16. Seats are limited to sign up now! If you can’t make it we will send you the replay. Sign up today: https://thephysicianphilosopher.com/Webinar

#23. How to have a Healthy Money Mindset
Our past experiences with money may have determined what we think about money now but doesn't need to impact how we think about it later. As someone who is fascinated by the psychology of money, I can understand why many doctors I talk to have a scarcity mindset when it comes to money. However, I also know that this mindset negatively impacts income and causes people to continue working more and as they try to grow their income. If you feel like you can't earn any more money, I'm going to help you break out of this negative mindset. Let's talk about two of the most common limiting beliefs I see doctors have about making money and get you to think about the abundance of your value. Also, I am hosting a webinar titled Physician Burnout: 3 Essential Concepts to Finding Freedom on 4 upcoming dates starting February 16. Seats are limited to sign up now! #22. 4 Reasons to Stop Apologizing Every Time You Say No
It’s Nice to know that Wilt Chamberlain and I have at least one thing in common: we both got over our need to please people. I used to feel responsible for other people's feelings. It's taken so long for me to realize being authentic to who I am means saying no, no matter how disappointed someone else is. That's why I want to share with you these 4 reasons you should stop being a people pleaser so that you can live your life. It's not possible for us to please everyone all the time, so why do we say yes to everything, even when deep down we don't want to? #21. Choose Curiosity Over Shame
“Every action you take is a vote for the type of person you wish to become. No single instance will transform your beliefs, but as the votes build up, so does the evidence of your identity.” Our human brains are so interesting. So when it comes to our routines, it’s easy to fall back into them after wanting to enact change. You work on making intentional choices. You may even make a resolution. You try not to give into your urges but then… BAM! All that good work seems to go to waste. Here’s the thing, though… your efforts aren’t wasted at all. Choose to be curious about the decision to fall back rather than shame yourself. Choose to treat yourself with the same dignity and respect you would for someone else. Learn how not to beat yourself up. Here are a few pieces of information that could be a deal maker for creating sustainable change when it comes to habit formation. #20. Building a Hell Yes Policy
When you say yes to one thing, you’re saying no to another. Are you making the right decision when choosing between the two? [emoji shrug goes here] Life as a doctor can constantly feel busy. You’ve got not only your work caring for patients, but you’ve got family, you’ve got side interests, your businesses, and a long to-do list for pretty much every hat you wear. I unapologetically sympathize with you. That’s why the Hell Yes Policy means what it means for me to pass along to you. This concept has helped me get through some tough times. Let it help you get through too. You don’t have to wait to join The Alpha Coaching Experience. Signup here: https://thephysicianphilosopher.com/waitlist

#19. New Year, New You - Building/Breaking Habits
What if you make 2021 a year where you actually accomplish your goals and stick to a new you? That’d be nice, right? Turns out you really can but… queue drum roll... there is a systemic approach to doing it. When it comes to habit formation, I always like talking about the concepts of System 1 and System 2 from Amos Tversky and Daniel Khaneman. You can read more about this in Khaneman’s book, which details their work called Thinking, Fast and Slow. The fundamentals include assessment, interpretation, and intentionality. At least that’s how I talk about it in this show. Through personal experiences, I share how my habits have brought me to a place of unexamined opportunity. I’m not perfect but I was able to rewire my brain. I give you several practical tips for you to do the same. Sign up > > One of my greatest joys is helping you, coaching you, and supporting you. So, I’m constantly looking for ways to do that including in my email. Join and subscribe here: https://thephysicianphilosopher.com/every-episode

#18. Faith, Beliefs, and Coaching
My faith was the reason for getting into coaching in the first place. But admitting my religious beliefs publicly usually does two things-- brings people closer or pushes them away. This episode is about having conversations with people who are different from us. It’s about removing the stigma, it’s about meeting people where they are, and it’s about loving our differences while we help each other be better people. Getting o’vr our differences in faith and belief helps us show up for others (and ourselves) in a way that doesn’t discriminate. When we remove the judgmental lenses we’re sometimes nurtured to have, we impact more, love fruitfully, and, at the end of the day, are better human beings. Are you currently facing your own mirror while meeting others where they are? Please let me know. I’d love to read your thoughts. Email me at editor@thephysicianphilosopher.com

#17. How to Deal with Haters
Online trolls. Gotta love them. Or, hate them. Whatever. As a blogger, I’ve experienced a lot of online hate. One post in particular brought some haters out into the light, but as someone who really cares a lot about the things they were chastising me on, it really hurt. I have a newfound appreciation for criticism and I want to share it with you as you get into the online space. If you’re doing something worthwhile and you’re piquing enough interest that someone talks about you in a negative light, the best way to get around it is to … learn how to deal with it. I’ve got a few solutions for you to follow. TL;DL (too long; didn’t listen) = Haters gonna hate. Don’t feed the trolls. #16. YOLO versus FIRE
“At the end of the road, we’ll all be happy.” That’s the mindset I’m falling into these days and I think it’s helped my wife and I find that pursuing FIRE is worthwhile. But, that’s not the case for a lot of doctors. The reality of our roles as physicians is that finality is ever-present. We see it in the emergency room. We see it with deceased family members. We even see it with our goals. Since everything in our lives is driven by our feelings, we might be getting in our own way to financial independence. There is a very reasonable road that you can save enough for tomorrow and have enough today. Listen as I dig into what this means. Who knows, maybe you’ll do some soul searching while you’re at it. This podcast is only a fraction of what I have going on at The Physician Philosopher. If you haven’t yet, check out the blog at www.thephysicianphilosopher.com. Have a question for the show? #15. Changing Jobs: 3 Tools to Help With the Decision
There’s a fundamental thought that our brain is powerful. When we have few facts and have perceptions without them, we tend to create stories that, in effect, hurt us. Get your thought work done and start creating new ones that serve you best. But, this takes very intentional work in mind and in body. I share a few things in this show on how to do the tough thought work so that you can change your circumstances, especially while changing jobs. Instead of hearing just me on the show, I’d love to share some space with you. Do you have a question for me that I can answer on the podcast? #14. Stop Trading Time for Money by Building Non-Clinical Income
Maybe you feel like you can help people beyond medicine. Maybe you have a creative outlet you’ve yet to dive into. Maybe you just want more freedom! You can always make more money but you can’t always have more time-- whether that be with your jobby or your side hustle. What it took me a long time to understand, I help you realize now about how not to trade money for time by building non-clinical income. Starting with the end in sight first and working yourself backwards is one of the best attempts you can make to improve your life. Are you ready to get your time back and create space in your life for freedom? Please say, yes! You deserve this kind of life. Good friends and fellow physicians, Drs. Leti and Kenji of Semi-Retired MD are opening their Zero to Freedom Through Cashflowing Rentals online course in December. #12. Finding a Way Out
For those that are burned out, morally injured, or otherwise overwhelmed - the answer seems to be to leave medicine, go part-time, and create non-clinical income streams. Why? Because we think those changes will make us happy. Or content. Or satisfied. Or fulfilled. BUT… If we don’t figure out why we are unhappy FIRST, and we change our circumstances, then we have a VERY HIGH chance of being unhappy after we make the change. #11. The Fear of Failure
When you fail, the easiest thing to do would be to give up. It’s too hard to keep going. It’s too hard to face reality. It’s too hard to pick up and pivot. But, is it really? Failure is a natural thing. It teaches us lessons that we otherwise would take for granted. This episode helps you acknowledge the importance of flipping the script on failure when it counts the most. If you have questions about coaching or about something you heard in a past episode, I’d love to hear from you and answer it on a future show. #10. It Is All Your Fault
Truth bomb... The feelings you have about work isn’t up to your spouse, your boss, the C-suite, or anyone else. Those feelings are 100% on you. So, if you’re overwhelmed and burned out right now, maybe you should ask yourself this: why is that? In this episode, I share one of the biggest misguided fails of doctors when it comes to the way we think about our circumstances, the past, and how we react to both. Don’t miss this one. #09. Everything is Coachable
There’s an interesting dynamic happening in the physician coaching space and it leans more towards women -- which is a great thing. As a dude, I think there is so much to gain as a man who finds a coach and values it. Dr. Junaid Niazi helps me explain why it’s important for men, among women, to get out of the burnout, shed the locus of control, face the side effects of a broken healthcare system, and expose themselves to the type of life they can have residing in their dreams. Join us as we address how everything is (and has always been) coachable. #08. Take the Edge off Without Alcohol
Buffers like alcohol reduce your ability to show up the way you want to with your family, at the hospital, and in life in general. It often takes mind work to avoid reaching for your favorite drink but what does that really look like and why? In this episode, I talk about a personal experience of choosing and then NOT choosing alcohol because of specific life events and the mental systems that help support/enable healthy change. Do you need help thinking through how to take the edge off without the use of buffers? I’d love to work with you. By now, you’ve heard of the Alpha Group Group Coaching Experience. #07. How to Stop Feeling Overwhelmed
Keeping up with the pace of life, work, and family sometimes means getting our priorities a little jumbled. We multitask to try to get everything in and everyone paid attention to. What we’re actually doing is creating a dynamic of overwhelm. Let’s stop for a moment and reflect on what we can do to make our lives better and our minds focused on what really matters. In this episode, I mentioned A Priori Planning and how it’s helped me streamline my life and my energy.
