Richard tells the story of Dr. Gary Fettke, an Australian doctor who removes diabetic feet. He has been treating his patients with a ketogenic diet, and in 2014 was silenced by the establishment. He is not allowed to talk to his patients about nutrition. If that wasn't maddening enough, an interview with Ivor Cummins reveals the diabetes research by Dr. Joseph Kraft in the 70's. Kraft discovered a method for detecting T2DM 20 years before a diagnosis. His research was effectively ignored. The good news is that you can leverage Kraft's research to figure out your risk of diabetes right now with a simple blood test. We're mad as hell, and we're not going to take it anymore! Also, Ivor does use a profane phrase (GD) toward the end of his interview, which we bleeped. Errata: Richard said that of the 54 Randomized Controlled Trials (RCTs) testing low carb vs low fat, that 43 showed that low carb was better, and 26 were significant. The name is in fact protected, and Pecorino Romano is only made in certain area. It's made out of sheep's milk, ergo the name "pecorino" in Pecorino Romano. The Italian word for sheep is "pecora". Its close cousins are other pecorino cheeses, perhaps the second more famous one that is made in Tuscany. But, not as salty and pungent as the Romano. They are milder and eaten as slices. While Pecorino Romano is often grated and shredded, and added to dishes. She writes: In our podcast, Carl asks me if babies *have* to be in ketosis to build brains, and I say yes, but that's not true. That is, the primary way that babies build fat and cholesterol in the brain in normal conditions *is* out of ketone bodies, though a small proportion is also made from glucose. I have a few references on this point in the talk transcript on my blog. The critical question is: What happens if there are no ketones to be had and only glucose? The answer seems to be that glucose will suffice. There is a rare inborn error of metabolism called HMG-CoA lyase deficiency, which prevents the body from making ketones. A paper by another brilliant Morris ( Cerebral ketone body metabolism ) reports that people afflicted with this have white matter abnormalities, but no noticeable loss of function, except of course, they can't go without food for long. This suggests that in cases where there are no circulating ketones, the glucose alternative pathway will take over, and get at least an adequate brain constructed. It occurs to me that children with this condition may be perfect candidates for the therapeutic use of ketones esters, provided the condition doesn't somehow prevent their use. "Given the importance of KBs [ketone bodies] as substrates for myelination, one might expect disorders of ketogenesis to be associated with cerebral white-matter abnormalities. Magnetic resonance imaging has, indeed, shown diffuse mildly increased signal in the white matter of patients with HMG-CoA lyase deficiency. Superimposed on this are foci of more abnormal signal. In most cases, multiple lesions have been present, varying in size from a few millimetres to large confluent areas; as well as in the cerebral hemispheres, they have been reported in the internal capsule and brainstem but not in the corpus callosum or cerebellar white matter. Despite the imaging abnormalities, most patients have had no neurological problems and normal or slightly below average intelligence. Protein Controversy

Richard Morris puts forth a hypothesis that explains three controversies surrounding protein. 1) How much protein should we eat? 2) If you fast do you lose muscle mass? 3) If you eat protein does it turn into sugar in your body? Experts disagree about the answers to these seemingly simple questions. Richard has done the math to explain how they are all correct under the right circumstances. This is a very technical discussion, but there is practical guidance that comes out of it, if indeed Richard is correct. As of this recording, it's just a hypothesis. More study and more input is required. Errata: Richard referred to a minimum requirement of 51g of glucose being required to run the brain, the actual figure should be 50g - he made a rounding error in his spreadsheet. Tom Naughton, Fat Head

Carl Franklin and Richard Morris interview the one and only Tom Naughton, software guy turned comedian turned Netflix star. His documentary, Fat Head, was first made in response to the movie 'Supersize Me' in which a perfectly healthy man ate 3 meals a day at McDonalds for 30 days to see what would happen to him. The results weren't good. Tom's response was to prove that he could also eat McDonalds' food for 30 days and stay healthy, even lose weight. During the process of making the film he learned about low-carb high-fat eating, ketogenic diets, and the like. Nowadays Tom is an advocate for the ketogenic lifestyle and is still poking fun at the ignorance that abounds in the world of health and nutrition. Eratta: Richard and Carl both mentioned that Beverly's husband who had the late night low glucose event could have eaten a small bit of fruit to get his blood sugar up. However, Andrew in our forum suggested a better option. A spoonful of coconut oil goes directly to you liver to be burned for energy, and if your insulin is low the fat will be partially burned, releasing ketones. Ketones support your brain's need for energy, meaning if you have circulating ketones then low glucose does not necessarily mean that your brain is running out of energy. Also, we incorrectly tallied Tom's quiz score at 5 out of 10. He actually only got 4 out of 10 correct. This week, Carl Franklin and Richard Morris get angry about how the medical establishment hasn't evolved along with science. We KNOW the chain of events that leads to type 2 diabetes (T2DM). Not only do we KNOW how to reverse it, but we knew in the 70's! Why after all this are most doctors still clueless about how to treat it? Richard tells the story of Dr. Gary Fettke, an Australian doctor who removes diabetic feet. He has been treating his patients with a ketogenic diet, and in 2014 was silenced by the establishment. He is not allowed to talk to his patients about nutrition. If that wasn't maddening enough, an interview with Ivor Cummins reveals the diabetes research by Dr. Joseph Kraft in the 70's. Kraft discovered a method for detecting T2DM 20 years before a diagnosis. His research was effectively ignored. The good news is that you can leverage Kraft's research to figure out your risk of diabetes right now with a simple blood test. We're mad as hell, and we're not going to take it anymore! Also, Ivor does use a profane phrase (GD) toward the end of his interview. We left it in for effect. Errata: Richard said that of the 54 Randomized Controlled Trials (RCTs) testing low carb vs low fat, that 43 showed that low carb was better, and 26 were significant. Carl Franklin and Richard Morris talk about the science behind heart disease, a component of cardiovascular disease. What is it? When did it start becoming a real problem? How did it happen? Errata: Richard mentioned the "number needed to treat" or (NTT) for statins is 141. But that is incorrect. The Number needed to treat to save one life for someone who has heart disease is 83. You need to treat 83 people over 5 years with statin therapy to save one life. The NTT to save one life for someone without prior heart disease is 8 because no lives would be saved no matter how many people without heart disease took statins. Carl mentioned that Mahler's bread is only 1 net gram of carbs per slice. There is some controversy around this. Some labels show 5 net carbs and some (including the label on my own loaves) show 1 net carb. Losing 290 Pounds with Tom Seest

Carl and Richard interview Tom Seest. Tom is a software developer from Illinois. In 1999 he injured his back falling down stairs. He was always active before the injury, completely stopping all exercise after. He put on weight. A LOT of weight. He estimates he was over 500 pounds. He started an Atkins diet in 2009 and was down to 490 by the end of 2010. He tried various combinations of low-carb, low-fat and low-calorie diets. In April, 2011 he took his first bike ride and went 2 miles. By the end of the month he was up to 20 mile rides. Since then he's logged 21,700 miles, and has cycled all over the US and in many other countries. He managed to drop 200 pounds with a combination of diet and exercise. In January, 2015 he started a ketogenic diet and has taken off an additional 90 pounds! We talk to Tom about his experiences coping with his situation. Errata: Carl mentioned that the fat you eat has no effect on insulin. This is effectively true but technically false. Dietary fat does raise insulin ever so slightly, but the effect is negligible. Ketones

Carl and Richard discuss Ketones. What are they? How are they produced? Are they safe? How can we effectively measure our ketone levels? Does it actually matter? All of these questions and more are answered on this episode of 2 Keto Dudes. Errata: We said we'd talk about consuming ketones but we never did. There is evidence that the mere presence of ketones in the blood stream is beneficial for controlling some kinds of cancer (listen to the cancer show from last week) however, taking ketones isn't a short cut to ketosis. It's like instead of going for a run you just cover yourself with sweat. The process of burning fat and creating ketones is where the magic happens. Carl mentioned that mitochondria are organelles in _every_ cell in the body. But there are some cells like red blood cells that lack a mitochondria - which is why they can only ferment glucose for energy and are unable to use fat for energy. Richard said that the Romans discovered that fasting cures epilepsy, but apparently a Greek doctor in the roman empire name Galen of Pergamon was just the first to write down that the Greeks had originally discovered the link noting thati n the book "Epidemics," Hippocrates describes the case of a man whose epilepsy was cured with drastic diet and fasting. Alcohol

Carl and Richard discuss alcohol, how it is metabolized, particularly by those eating a ketogenic diet. They discuss the carbohydrate content of typical drinks, and share a couple yummy keto-friendly cocktail recipes. Errata: Richard said he'd had 3 Bottles of Moët before the show - but it was actually 3 GLASSES of Moët. Also Richard said he didn't know of research substantiating Alcohol inhibiting gluconeogenesis and 5 minutes after we finished recording he found one from Hans Krebs (linked below). Update: May 11, 2016: Carl did an n=1 study on himself by following a 22/2 intermittent fasting pattern for 3 weeks. He ate only dinner, but had drinks with and after dinner. The result was a big plateau. No major weight loss. He then did a 2 day fast and started eating his one meal at lunch time (with no alcohol) saving the drinks for the evening. The results were positive. He started losing a pound a day. His hypothesis: When you drink alcohol your liver stops metabolizing food and focuses on the alcohol. Once all the calories are extracted from the alcohol the liver goes right back to metabolizing food, but now your caloric intake has increased and some of the calories from the food will not be used, and some of the fat will be stored in the fat cells. By giving the body time to process the food intake, you allow the liver to do it's job. More fat gets burned. Errata Richard mistakenly referred to the ancient greeks discovering that Diabetes is related to sugar by tasting it .... actually that's not true. In 1674, it was English physician Thomas Willis discovered that the urine of a diabetic Patient was "wonderfully sweet as if imbued with Honey". The ancient greeks however first noticed that people who had diabetes had insatiable thirst and peed all the time - so the name diabetes is greek for "Siphon". They have a sense of humor those ancient greeks.
