Skincare Decoded: Complete Skincare Course for Experts

On-Demand Workshop on Skincare Routine Steps Welcome to The Slide Show with Dr.TWL Episode 2, where we present skin science in bite-sized formats. Today we will be covering the first, fundamental step in skincare routines, makeup removal. Makeup removal What are the two main formulation options for makeup removers?The first is a micellar formulation. The second, an oil-based cleanser or an emulsion, sometimes known as a “milk” cleanser. I will go through the pros and cons of each but first let us define some terms. How does micellar water work? Micellar water works by hydrophobic (water-hating) and hydrophilic (water-loving) properties of a micelle. The makeup residue is attracted to the water-hating aspect, this is best thought of as a ball that is wrapped up which continues to roll on skin grabbing the residual pigment and dirt. To remove the ball which is a micelle, you use a cotton pad. Friction or rubbing is inevitable and this can be harsh for sensitive skin. What is the downside to oil cleansers? Pure oil cleansers are often too greasy, my choice is an emulsion or a milk cleanser. Milk cleansers are oil in water formulations, the oil component dissolves makeup pigments and removes excess sebum. The “like for like” principle here is that oil soluble pigments are dissolved in a similar substance—an oil in water emulsion. What is the difference between emulsion vs oil cleansers? The benefits of an emulsion are that it is less greasy than pure oil formulations. It is also effective as a humectant if formulated with moisturising ingredients. When you physically rub off the makeup on a cotton pad, the oil component protects your skin. It acts as a barrier between the cotton pad and skin, hence reducing friction unlike with micellar solution. The reason is because excessive growth of p acnes occurs in individuals who produce a certain type of oil- defined by its physical and chemical composition. Facial Mists for Skin Barrier Repair

Our skin’s source of water comes from the deeper layers of the skin. When the skin is initially exposed to a dry environment, our skin reacts by delivering more water to the skin surface. The surge of water supply eventually plateaus (given there is only so much that the deeper skin layers can delivery), and this ultimately leads to dry skin. A facial mist delivers water content to the skin surface to preserve hydration levels. While some of the facial mist will be lost to the surroundings, it still helps to keep the skin moisturized. A mist also enhances skin viscoelasticity by softening the skin layer and maintaining skin moisturization. You can expect greater elasticity or resilience with frequent spritzing of your favourite mist. Skin shedding is the body’s natural exfoliation mechanism which allows new cells to regenerate, revealing radiant, healthy skin. The loss of cell energy within the mitochondria also means that the body will be unable to clear excess melanin pigment, resulting in accumulation and the development of hyperpigmentation. A Conundrum: The Science Behind Hyperpigmentation & Sensitive Skin

Hyperpigmentation is a key feature of photoaging, along with the development of wrinkles, loss of skin elasticity and facial volume. Pigmentation isn’t just a cosmetic concern—it can be a sign of underlying inflammation. Post-inflammatory hyperpigmentation for instance arises from conditions such as eczema or other forms of dermatitis, all of which are part of what is widely referred to as sensitive or reactive skin. Acne scars are also a form of hyperpigmentation, beginning with post-inflammatory erythema or redness immediately after acne resolves. This redness is a sign of inflammation, which later becomes pigmentation as the melanocytes begin to produce melanin. Melanin deposited in the superficial and deeper layers become what we know as hyperpigmentation.  Traditionally prescribed tyrosinase inhibitors such as hydroquinone reverse this process by blocking the melanin synthesis pathway. However, hydroquinone is also associated with skin irritation which can occur even with short term use. Sensitive skin for example does not respond well to hydroquinone therapy—it is possible for flare ups to occur necessitating the use of topical steroid therapy. For instance the 3-in-1 hydroquinone-retinoid-steroid formula known by its trade name Triluma was specifically created to counteract these side effects during the treatment of melasma. Such an approach isn’t a solution either—those with sensitive skin are susceptible to side effects of irritation, caused by barrier disruption.  Is it possible to treat hyperpigmentation without the use of prescription topicals? Transhumanism, aesthetics & fractals—the science of beauty is not what we are led to think it is

The rich and beautiful have it all, don’t they? Celebrity suicides draw attention to pervasive mental health issues that are equally indiscriminate—plaguing the young and attractive—those regarded by society as the most successful. Is it possible that the practice of cosmetic dermatology breeds the vicious cycle of insecurity in body dysmorphophobia rather than addressing it? In this podcast I share some of my reflections that led to my decision to leave the practice of cosmetic dermatology back in 2019. Normative perfection does not exist. Does it? The relationship between the material and the mental world was referred to by Gustav Fechner as the realm of psychophysics, also known as “die geistige Welt” in German. A literal translation to “spiritual universe”.  As the pioneer of modern psychology, he proposed a set of metrics which could be used to statistically define the experience of beauty—also what makes one “fall in love”. Contemporary models examining the psychology of beauty refer to it as an undefined aesthetic response. We debate that beauty must be universal since we prefer familiar objects, yet philosophers hypothesise that beauty must possess an inherent element of surprise. Psychologists also acknowledge that the experience of beauty requires an element of cognition—it’s also paradoxically ascribed to pleasure, experienced as a visceral sensation. These diverse attributes are consistent with biological phenomena—complex, interactive processes that work towards a collective purpose.  In the age of AI, will we finally achieve perfection? Or is there an insidious force already working behind the scenes. Join me in this episode as I unravel the dangers of following the crowd, especially in the business of beauty.

Skin Streaming 101

2023 saw TikTok skincare trends of skin flooding and later, skin streaming take centrestage. What is the science behind this? Now, if you are a skincare minimalist, the great news is that these two concepts are actually synergistic—here’s my take adapted for teen skin. First, some quick definitions. Skin flooding involves layering skincare i.e. serum, lotion, cream, ointment ala K-beauty multi-step skincare routine style whereas the concept of skin streaming refers to just sticking to a “streamlined” as-needed philosophy for your daily regimen. Practically speaking, these trends make no material impact on what dermatologists actually recommend, based on the core principles of: 1. Choosing efficient skincare actives i.e. cosmeceuticals backed by evidence based medicine 2. Not exposing your skin to unnecessary, multiple irritating actives, applicable in the context of acids such as AHAs, BHAs, retinols, retinoids which can cause skin sensitisation 3. Respecting the skin barrier overall How do you carry out skin streaming? That being said, I feel the concept of skin streaming is most relevant for those who are relatively new to complex skincare regimens and may be confused about where to begin in the first place. Flash Skin Tips #3 Beauty Sleep is Seriously Underrated

I am obsessed with sleeping 8-9 hours a day. This is it—the best skincare tip I have for you is to SLEEP. It’s essential to being alive, by the way. And I don’t meant to sleep at 2 am and wake up at 2 pm. To be beautiful (and healthy) you need to follow the circadian rhythm. Like the cicada…you know? Bugs? 1. Beauty Sleep Is beauty sleep real? I guarantee it. During the sleep cycle, skin cells work to repair damaged DNA. We all know about the sunscreen doctrine—how it prevents UV rays from damaging skin cells by preventing oxidative stress. But did you know that skin does repair its own damaged DNA? This is why the older you get, the higher the risk of developing skin cancer. Biological and chronological aging slows down the DNA repair process. Sleep helps to rejuvenate cell mitochondria, which are the powerhouses of skin cells. There is scientific evidence that backs up skin dullness and lack of radiance in sleep-deprived skin—and it isn’t psychological.  How I wake up with good skin • Sleep by 9 and wake by 6 am Your biological clock follows the circadian rhythm—which is the sun rise and sun down pattern. Our body’s physiological processes follow that too. This ensures your cells are optimally rested and can focus on fighting the bad guys (pollution, stress, UV damage) in the daytime • Choose cooling bedlinen If you live in tropical humidity like I do (Singapore, land of crazy hot), you know your body heats up when you sleep. Pure cotton works well, choose higher thread counts which are gentler on skin. Your pillowcase makes a difference—use silk pillowcases or in my case I use one with copper silk which gives extra anti-aging skin benefits.  • Consider sleeping without A/C This may be heretical for some—I used to be an aircon worshipper too. But it’s also true that A/C is bad for those with allergies, asthma and eczema. It dries out the skin barrier and it’s no different when you sleep. Sleep with a fan—that’s better for your skin and healthier for the environment too. Your skin will wake up feeling more moisturised (without sleeping masks, it’s free!) • Have a sleep ritual  Sleep hygiene essentially means having a bedtime ritual. I highly recommend  1. Skincare routine 2. Incorporate massage i.e. jade roller to relax facial muscles, increase blood circulation, tone the face 3. Wearing comfortable sleepwear, include sleeping eye masks which can further help you get a good night’s rest 4. Aromatherapy (if you enjoy essential oils) can improve overall wellbeing and aid relaxation 5. Bedtime relaxing activity: for 15-20 minutes journal, practise gratitude, coloring etc 6. THE SKINCARE SCAM All sleeping mask formulas are the same as moisturisers, as these are leave-on rather than wash-off ingredients. What Is My Skin Type?

Flash Skin Tips #1 : A Dermatologist's Guide, Use This Test To Tell Your Skin Type •CLEANSING BEGINS WITH UNDERSTANDING SKIN TYPES• Choosing the right cleansing regimen is the most important part of any skincare routine, do you agree? I’m Dr Teo Wan Lin, board-certified dermatologist, founder of clinical skincare brand Dr.TWL Dermaceuticals and international beauty KOL. In this sharing, I highlight why the first step to a solid skincare regimen is knowing your skin type. I teach you how to quickly tell, with a quick test you can do at home. What Is My Skin Type? Dry: skin that produces less sebum than normal skin, individuals with inherited DNA defects in filaggrin gene causing eczema, previous skin irritation with cleansers/skincare Oily: skin that produces more sebum than normal skin (an overproduction of sebum is known as seborrhea) Normal (scientifically known as ‘eudermic’): well-balanced and healthy skin, with good production of ceramide as part of the skin barrier Combination: a mix of skin types in the T-zone and cheeks How Do You Determine Your Skin Type? The best way to do this is simple and you can even do it at home with the following steps: Wash your face and let dry for 30 to 60 minutes. Dab a tissue on the T-zone (nose, forehead) and cheeks. If you feel that only your nose is oily, that means you have combination skin. If you are oily on your nose and cheeks, you have oily skin. If both your T-zone and cheeks are dry, it means you have dry skin. To check if you have dry skin, you may use the following checklist as well. Do you have: Dry itchy skin which may be worsened by exposure to strong soaps, sweat, dusty environments, pet fur, pollen mould and carpets A personal history of atopy, which encompasses hay fever, sensitive nose (allergic rhinitis) asthma and eczema Family members with atopy Skin redness, flaking, itchiness, stinging whenever exposed to a new environment or to certain skincare or makeup products If your answer to any of these questions is yes, you have dry (type 1) skin. For dry skin types, it is recommended that you moisturise frequently with products that have moisturising ingredients such as ceramide, glycerin and squalane. For oily skin types, you will still require a moisturiser but perhaps look for hyaluronic acid-based ones. You will also be able to tolerate more drying ingredients such as glycolic, lactic and salicylic acids. For normal skin types, it is advised for you to change your routine up based on the weather. In warmer months, you may add dryer skincare products and in the colder months, you would need extra moisturisation. While your skin type is determined by your genetics, your skin may be affected by other variables such as environmental factors and age. Now that you know your skin type, you can incorporate skincare products which are suited to your skin to achieve the best results for your skin. My fascination with the realm of the imagination as a dermatologist began with my observations of body dysmorphophobia. In this case, sufferers experience horror and self-loathing in a purely imaginary way – yet it is so real it manifests with concrete reactions like continual body-modification attempts with plastic surgery and persistent depression or even suicide. Body dysmorphobia is a classic example of imaginations gone wrong. Sufferers see a distorted vision of themselves, that is no longer consistent with reality. Yet, there is a way we can try to perceive this condition: their distorted vision of themselves directly mirrors their self-image – which is a product of their thoughts, emotions and their personalities. Hyperpigmentation is often associated with aging. However, there are actually many different types of pigmentation that dermatologists encounter. While not all forms of hyperpigmentation are the same, there are a few common pathways that effective treatment options can target. Melanocytes are the cells that produce pigment, known as melanin. These 3 ingredients work effectively and holistically to regulate melanocyte activity: tranexamic acid, kojic acid and niacinamide. In the formulation of an effective serum to treat hyperpigmentation, addition of 5% sulphonic acid has an added benefit of increasing skincare absorption by exfoliation. Beauty is a topic of fascination throughout millennia. Gustav Fechner, the pioneer of modern psychology proposed a set of metrics which could be used to statistically define the experience of beauty. Known as psychophysics, he examined the relationship between the material and the mental world, known as “die geistige Welt” in German, which translates literally to the spiritual universe. In contemporary models involving the psychology of beauty, it is primarily regarded as an undefined aesthetic response. There have been diverse interpretations of beauty as an experience. Philosophers have hypothesised that beauty must possess an inherent element of surprise. It’s also been ascribed to pleasure as a visceral sensation. Psychologists have debated that beauty itself innately must be universal, which may explain our preference for familiar objects. Interestingly, Immanuel Kant famously proposed that beauty, in contrast to sensuous pleasures, required thought. Is beauty an emotion after all? Drawing an example from the eastern art of ink painting, she shares that the ancient chinese idiom of a lotus being a symbol of moral perfection arising out of imperfect muddy waters is an example of a cultural viewpoint that regards morality and a virtuous character as the highest forms of beauty.
